AVAILABILITY

Mark YES or NO for ALL timeslots, do not leave any blank. Please elect timeslots according to your level of experience and/or desired placement. Please note these times are "proposed" times only, not all times will necessarily be available. Therefore it is important you mark YES to as many times as you can. Please place an * next to your preferred timeslot(s), this will be taken into account.

ENROLMENT CONDITIONS

By signing this enrolment form you agree:
1. Per term FEES (incl gst) as at the date of signature are:
- 1 class per week for 7 weeks = $440
- 2 classes per week for 7 weeks = $640
- 3 classes per week for 7 weeks = $840 (existing students only) 
 2. You will comply with the terms of payment as follows: A minimum deposit of $100 is required to secure your place in the program with the balance due at or prior to the commencement of your first class. (If desired, fees may be paid in full in the first instance, instead of deposit + balance.)
3. Course fees are only refundable in the following instances:
3.1 Up till 14 days prior to term commencement.
3.2 In the event that the studio is unable to provide the specified class at a time matching the availability supplied above on this form.
4. The studio is not responsible for late enrolment cancellations or unforseen changes to your availability. Cancelation prior to course commencement (less than 14 days prior) will result in forfeiture of your deposit only. Cancelation after course commencement date (Mon Aug 9th 2010) will result in forfeiture of the full term fee. The course is deemed to have commenced as at the first class of each term irrespective of each individual student's own class schedule or their attendance at the first class.
5. Each student progresses according to their own rate of development. Individual progress may vary and the studio offers no guarantees with respect to the speed at which each individual student will progress.
